When will I find out if I passed? You will find out the day of your exam before you leave if you have passed or not. Though it could be up to a month before the state has your certification listed online.
How do I find my official state certification? After passing your exam, you can find your certification on the state’s website at Oklahoma Nurse Aide Registry Search. Type in your first and last name, and it will come up. Typically, it takes about 1-2 weeks for it to show though the State has 30 days to list the certification on the Nurse Aide Registry.
What will my Certification be called? Your Certification will be based on the Training your School/Facility provided. Once you have successfully passed your exam your certification will be listed as LTC or LTC/HHA depending on if your training included HHA training. LTC is what the state calls CNAs, and HHA stands for Home Health Aide.
Who do I contact regarding issues with my official certification? For any questions regarding your state certification, please call the nurse aide registry at (405) 426-8150 or email them at NAR@health.ok.gov.
Who do if I fail my test? If you your test you will have to reschedule a retake just like scheduling your original test. For CNAs just find the and schedule the parts you need to retake whether the skills or knowledge portion. For CMAs or ACMA tests you schedule your retake just as you did before by selecting and time and location for your test.
Important Note on Materials: You are required to bring your Original Training Verification Form. Copies or digital pictures of this form will not be accepted under any circumstance. If you fail to bring this original form or a valid government-issued photo ID, you will not be allowed to test, and you will have to reschedule and repay the full exam fee.
Required ID: Bring one form of government-issued photo ID (such as a driver’s license, state ID, or passport).
Required Testing Documents: You must bring your original copy Training Verification Form from your Training Facility. You must also bring a valid form of government ID (Identification)
Non-Citizen Documentation: If you are not a citizen you must bring appropriate documentation showing legal status for the United States.
Be On Time: Plan your route carefully to ensure you arrive at the correct testing location.
Late Policy: If you are more than 5 minutes late, you will be locked out, barred from testing, and required to completely reschedule and repay the fee.
Rescheduling Reminder: If you need to change your test date, you must provide at least 72 business hours’ notice. Failure to do so results in forfeiting your fee, and you must pay again to book a new date. No refunds are given for any reason.

Prerequisites for Testing: To be eligible to sit for the Advanced CMA-Insulin exam, candidates must currently be certified in Oklahoma as a Certified Medication Aide (CMA), hold a current Oklahoma Nurse Aide certification with no abuse notations, and have successfully completed a state-approved advanced training program.
Required Testing Documents: On the day of your exam, you must present your completed OSDH Form 504 from your Training Facility documenting that you have finished the training requirements for Advanced CMA-Insulin Administration. You must also bring one form of a government-issued photo ID (e.g., driver’s license, state ID, passport).
